Artificial Intelligence (AI) is impacting many areas of life, but its integration with medical technology poses several ethical concerns.
While AI certainly has the potential to revolutionise diagnostics, treatment planning, and patient monitoring in the mid to long term, some healthcare providers remain uneasy, fearing that the technology could undermine the very foundations of trust and transparency that are non-negotiable in the delivery of effective healthcare.
So, what are the main ethical concerns about using AI in healthcare, and what are the key issues of which healthcare providers should be aware?
Recent data breaches in the NHS have highlighted the risk to sensitive patient information, particularly in systems – such as AI – that require large amounts of data to function. Ensuring robust data security is vital, not only to safeguard patient privacy, but also to prevent the erosion of trust among patients that could deter them from seeking advice. Healthcare providers who integrate AI into their medical technologies must implement proven cybersecurity measures to protect patient data and maintain confidentiality. Unfortunately, many of the cyber security risks arising from AI are still unknown, and existing cyber security platforms are not well set up to deal with problems arising from, say, an AI having its training data hacked or corrupted, or its control mechanisms taken over in some way.
Generative AI technologies present unique and well documented challenges in terms of bias and discrimination, as they can inadvertently perpetuate existing biases present in the data they are trained on, leading to discriminatory outcomes. This could easily lead to inequitable outcomes in healthcare environments. For example, an AI system might provide less accurate diagnoses or treatment recommendations for certain racial or socioeconomic groups, exacerbating existing health disparities and widening the gulf between minority groups and healthcare professionals. Healthcare providers must ensure that generative AI systems are trained on diverse and representative datasets and to continuously monitor and mitigate any biases that may arise.
Informed consent underpins ethical medical practice, yet obtaining it can be complex in the context of AI healthcare technologies. Patients need to understand how their data will be used, the benefits and risks of AI-driven interventions, and the potential for data sharing with third parties. This requires clear and transparent communication from healthcare providers to ensure that patients can make fully informed decisions about their care.
The rapid pace of AI development often outstrips the creation of regulatory frameworks and informal best practices designed to govern its use. To prevent misuse of AI technology and to protect patient rights, healthcare providers must navigate this evolving landscape, ensuring that their use of AI complies with existing regulations while also advocating for updated and comprehensive guidelines.
Many AI systems function as ‘black boxes’, meaning their decision-making processes are not transparent or easily understood. A lack of transparency can undermine trust in AI-driven healthcare solutions, as patients and providers may be unable to understand how certain conclusions or recommendations are reached. Developing explainable AI systems that provide clear reasoning for their decisions is essential to maintain trust and ensure the accuracy and fairness of medical interventions.
Finally, the integration of AI into healthcare also raises questions about legal liability. Determining responsibility for AI-driven medical errors is complex, involving software developers, healthcare providers, and researchers. Clear guidelines and legal frameworks are, therefore, necessary to address these issues and ensure that patients have recourse in the event of harm caused by AI systems.
